Application for a Civil Marriage Ceremony

This version of the Civil Marriage Ceremony application form must only by used if you have not yet submitted your payment at the Service Oshawa counter, and you would prefer to submit your payment by credit card. We will email you a link within 2 business days to process your payment by credit card, once we have confirmed your preferred date and time are available.

  • Do not use this form if you do not wish to pay by credit card. If you would prefer to pay by cash or debit, please visit Service Oshawa in person at City Hall (50 Centre St. S., Oshawa, ON L1H 3Z7). We are open Monday to Friday 8 a.m. to 6 p.m., Saturday 9 a.m. to 1 p.m. 

  • Do not use this form if you have already paid for your ceremony at the Service Oshawa counter.

    If you have already submitted your payment at the Service Oshawa counter please use this version of the form: Civil Marriage Ceremony - Service Oshawa counter payment, to apply for your ceremony.  You will need your receipt number in order to complete this version of the application form.

This form may contain personal information as defined under the Municipal Freedom of Information and Protection of Privacy Act. This information is collected under the legal authority of the Municipal Act, 2001, S.O. 2001 c.25, as amended. This information will be used and maintained by the City of Oshawa for the purpose of scheduling a Civil Marriage Ceremony. Questions regarding this collection may be directed to the City’s Information Access and Privacy Officer, City of Oshawa, 50 Centre Street South, Oshawa, Ontario, L1H 3Z7, 905-436-3311.

Marriage Licence

Note: It is not mandatory that couples have their Marriaige Licence prior to booking a Civil Marriage Ceremony with the City, however an original Marriage Licence must be provided to the Officiant at least three business days prior to the ceremony.

* Please note, if you are bringing a photographer/videographer and/or ASL interpreter to your ceremony, they will count as part of your total guest count.

The couple, their two witnesses, and the officiant do not count towards the total guest count.
